Cancellation

Unless you purchased our Emerald level of cover, you are covered for cancellation under your Insurancewith policy. One of the main reasons travel insurance is so necessary is to protect you and your finances against the unexpected cancellation of a holiday and the loss of any pre-paid costs.

If you have had to cancel your trip and would like to make a claim, you will need to fill out this claims form. The documentation checklist on the front page will list all information and evidence that we will need in order to assess your claim. Please complete the form as fully, truthfully and accurately as possible in order for our dedicated claims assessors, Travel Claims Facilities, to process your claim as quickly as possible.

Medical Expenses

Experiencing a medical emergency abroad can be extremely scary, not to mention potentially expensive. If you’ve contacted our Emergency Assistance team prior to receiving any form of treatment, they will have liaised with the treating facility to ensure that you get the best treatment. Insurancewith cover both in and outpatient treatment in the event that you need medical treatment during your holiday.

If you have needed medical attention abroad and would like to make a claim, you will need to fill out this claims form.The documentation checklist on the front page will list all information and evidence that we will need in order to assess your claim. Please complete the form as fully, truthfully and accurately as possible in order for our dedicated claims assessors, Travel Claims Facilities, to process your claim as quickly as possible.

If you have to come home early because of an illness or an injury sustained on your holiday, you should also complete a curtailment travel insurance claim form (below).

Cutting short your trip (curtailment)

If you’ve had to cut short your holiday, and have alerted our Emergency Assistance team before booking any return travel, you will need to submit this claims form.

Please complete the form as fully, truthfully and accurately as possible in order for our dedicated claims assessors, Travel Claims Facilities, to process your claim as quickly as possible.

Our Emergency Assistance team will need to be alerted to any circumstances which have led to you needing to cut short your trip so that we can ensure you are covered.

Personal possessions, personal money and travel documents

If you need to make a claim for lost, stolen, damaged or delayed personal possessions, you will need to submit this claims form.

Please complete the form as fully, truthfully and accurately as possible in order for our dedicated claims assessors, Travel Claims Facilities, to process your claim as quickly as possible.

Please note: all claims for personal possessions are settled based on the second-hand value of any items you claim for, and not their replacement or sentimental value.

It is required that you obtain a police report in the case of any lost or stolen items in order to make your claim valid.

Similarly, you should obtain a report from the airline or other transport provider in the case of making a claim against delayed or damaged personal possessions, confirming the delay or the damage.

Departure delay

Our policies will cover you for any extra expenses that may leave you out of pocket after a delayed departure of over 12 hours.

If you have experienced a departure delay and would like to make a claim, you will need this claims form. Please make sure you obtain written confirmation of the delay by the airline or other transport provider. The documentation checklist on the front page will list all information and evidence that we will need in order to assess your claim. Personal Accident

A trip can quickly turn into a disaster should you have a serious accident resulting in a lost limb or permanent disablement. If you need to make a personal accident claim, you will need this claims form to submit to our dedicated assessors.

Personal liability

Obviously we all want our holidays to go to plan, but accidents do happen and they can quickly turn very serious if you are held liable for causing injury to someone else, or damaging their property.

If you need to make a personal liability claim, you need this claims form.  The documentation checklist on the front page will list all information and evidence that we will need in order to assess your claim. Please complete the form as fully, truthfully and accurately as possible in order for our dedicated claims assessors, Travel Claims Facilities, to process your claim as quickly as possible.

Access to medical records

As a condition of your travel insurance policy it is important that where applicable, for example in the event of a claim, you must agree to allow us to have access to your medical records so we can verify your claim.

Whilst you can refuse consent, it may mean that we are unable to deal with your claim.
You are authorised to see the report, should you wish, before it is supplied to us or our agents, or at any time during the six months after that.

Should you disagree with the contents of the report or consider it to be misleading, you may ask your doctor to amend it. If your doctor disagrees, you may add your own written comments.

The doctor is allowed to withhold all or part of the report from you if they think that this would be in your best interests, or that of others.

At no time will the report be sent to URV or their agents without your consent.
